New Client Paperwork

WOI provides you the opportunity to complete the initial paperwork prior to coming in for your first visit. This way you can spend more time connecting and discussing the presenting problem and other aspects of you or your loved ones growth and well being. This section contains the needed initial paperwork to start therapy for individual, family, and couples sessions. You do not need to fill out the release of information form prior to your initial visit. This form is usually completed as a needs only element.

For marriage and couples therapy please couple the couples-gottman-assessments 5 page document in addition to the others.

For group counseling the paperwork will be provided on the first day you attend the group.
